I provide physiotherapy services directly in the homes of residents in London, Ontario, and surrounding communities. By working within your home environment, I tailor assessment and treatment plans specific to your mobility needs and the requirements of your space, ensuring that your recovery is practical and effective. In addition to delivering hands-on care, I assess your living space to optimize it for safety, focusing on fall prevention strategies and ergonomic adjustments to help reduce the risk of injury. My goal is to ensure that your home becomes a supportive space that aids in your long-term health and well-being.

Virtual Physiotherapy
I offer secure and confidential online physiotherapy sessions to residents across Ontario, providing the flexibility to receive expert care from the comfort of your own home. During these sessions, I perform detailed mobility assessments and develop tailored treatment plans, ensuring your needs are met even in a virtual setting. I also assess your home environment to optimize it for safety, helping you prevent accidents and support your recovery process. In addition, my virtual appointments can include personalized exercise programs, posture correction, pain management strategies, and ergonomic advice for your workspace.

As a sole proprietor, Daniel operates independently, ensuring personalized care for all clients. While external consultants or agencies such as bookkeepers, accountants, insurance companies, and legal professionals may have limited access to personal information in the course of their duties, their access is restricted, and they are required to sign confidentiality agreements.
WHY IS PERSONAL INFORMATION COLLECTED
As a healthcare provider, Daniel collects, uses, and discloses personal information to provide effective treatment and rehabilitation services. He collects information such as health history, physical condition, and social details to assess your healthcare needs and provide appropriate care. Daniel may also maintain health records to track progress and changes over time. In rare cases, he may collect information without explicit consent in emergencies or where consent is implied.
RELATED REASONS FOR COLLECTING PERSONAL INFORMATION
Daniel The Physio may use personal information for purposes other than direct treatment, including:

• Confidential documents are transmitted in sealed, labeled envelopes via reputable couriers.
• Electronic transmissions are anonymized or encrypted when necessary.
• External consultants and agencies sign privacy agreements to ensure confidentiality.
RETENTION AND DESTRUCTION OF PERSONAL INFORMATION
Daniel The Physio retains client records for at least ten years before securely disposing of them. Paper files are shredded, and electronic records are permanently deleted. If hardware is discarded, hard drives are physically destroyed to prevent data recovery.
ACCESSING YOUR PERSONAL INFORMATION
You have the right to request access to your personal information. Daniel The Physio will assist in identifying relevant records and explain their content if needed. Requests must be made in writing, and a fee may apply to cover administrative costs. If your records are older than one year, an additional fee may be required for retrieval from storage.If Daniel The Physio cannot grant access, an explanation will be provided. If you believe any information is incorrect, you may request a correction. Daniel The Physio will update factual errors and notify third parties as necessary. If Daniel The Physio does not agree that a correction is required, he will document your request and include it in the file.
